The 29-year-old, who rejoined Palace on a free transfer from Strasbourg earlier this week, needed just three minutes after coming off the bench to make an impact.
Replacing Tyrick Mitchell, who had already scored twice, Chilwell reacted quickest when Fulham failed to clear a Palace attack and fired home to complete the comeback.
The victory handed Pierre Sage his first win as Palace head coach following his summer appointment, while Fulham's difficult start under Alvaro Arbeloa continued with a third consecutive Premier League defeat.
Fulham made the brighter start and opened the scoring through teenager Josh King, whose effort took a deflection on its way into the net.
Palace responded through Mitchell, who was released by Eddie Nketiah before beating Bernd Leno from a tight angle to make it 1-1.
The hosts restored their advantage before half-time when summer signing Cesar Palacios scored his first Fulham goal following his move from Real Madrid.
Palace emerged strongly after the break and Mitchell was again at the heart of their response.
The wing-back drove forward before seeing his initial attempt blocked.
Yeremy Pino's follow-up was then cleared by Gonzalo Garcia, but Mitchell continued his run and managed to divert the ball into the net for his second goal of the afternoon.
It was the first brace of Mitchell's senior career and brought Palace level for a second time.
Sage introduced Chilwell with less than 20 minutes remaining, and the former Chelsea defender made an almost immediate impact.
Anan Khalaili's effort was blocked by the Fulham defence, but Chilwell was perfectly positioned to collect the loose ball and fire home the winner.
